Service delivery design
  • Children’s Therapy Improvement Project. This project was conducted for two Tasmanian Departments: Department of Health and Human Services and Department of Education. It involved a review of the current service system and a service modelling exercise. The project sought to develop a more equitable and child-centred delivery model of therapy services to preschool and school-aged children and their families. It involved extensive consultation with government agencies, schools, non-government agencies and hospitals.
  • Development of 5 Community Transport Strategies: two in metropolitan Melbourne and three in rural Victoria (including local governments: Colac Otway, City of Greater Geelong, La Trobe, City of Monash). The projects developed integrated and coordinated transport models. We achieved community input and feedback through community consultation in a variety of locations, using a number of different consultation techniques. All key community-based organisations were engaged throughout the project, as were commercial operators. The models were developed with key stakeholders, encouraging a cooperative approach between community-based providers and commercial operators. Action plans were included.
  • Work with Tasmania Department of Health and Human Services as part of the redevelopment of SAAP services for SAAP IV:
    • Development of a common assessment tool for use by all SAAP agencies in Tasmania. Developed through extensive consultation and workshopping with the sector;
    • Development of a training syllabus based on existing accredited training packages;
    • 12-month evaluation of the implementation of the common assessment tool through an action research approach with SAAP agencies;
    • Development of a brokerage model for implementation within the SAAP sector;
    • 12-month evaluation of the implementation of the brokerage model through a participatory approach with SAAP agencies.
  • Youth and family services in the Shire of Campaspe, Victoria. The project identified the outcomes required to improve the health and well-being of families and young people living in the Shire and developed appropriate service models to bring about a more integrated and coordinated service delivery system.
  • A review of After School Child Care was undertaken for Indigo Shire. This project provided advice to the Indigo Shire Council on a model of operation to assist with improving the quality of service and the development of a viable budget framework. Current best practice in the sector was identified and advice was provided in relation to: policy and procedures documents; the roles and responsibilities of staff and management; marketing strategies; and a service model.
  • Review of Food Services – Shire of Yarra Ranges: Services were mapped and the needs of the HACC target group were identified. Successful consultation occurred with a wide range of stakeholder groups across the Shire. Changes to the service delivery model were developed.
  • A review of two community services programs for Albury City Council (The Yellow Bus – a support service for children at risk, and Family Support Service). The project involved a review of the services, consultation with staff and service users, and a literature review of current good practice. An integrated service delivery model was developed.
Needs Studies
  • A Health Needs Study was conducted for a local Community Action Group and the District Hospital. It included 12 rural and remote communities in the Mallee, Victoria and the Western end of the Riverina in NSW. Community health and well being needs were identified through review of data, survey of service providers, a community survey and community consultation. An action plan was developed collaboratively with key stakeholders, service delivery models developed and a submission for funding to the Commonwealth Government prepared.
  • Needs Analysis of aged people in Lilydale district, an outer suburb on the fringe of Melbourne (commissioned by Shire of Yarra Ranges). Consultation occurred with a wider range of stakeholders, many of whom were of CALD background. The study was used to help inform Council and service providers plan for the future and consider a range of service delivery responses.
  • Aboriginal Home and Community Care (HACC) needs study in the Riverina area of NSW. This project, conducted for Home Care NSW, gauged the specific needs of the Aboriginal population and developed Koori-sensitive service delivery models for the future. These models were developed in consultation with the Aboriginal elders identified as our liaison people and the steering committee and informed by lessons other Aboriginal-specific services in NSW and Victoria.
Strategic planning
  • Development of a joint Municipal Public Health Plan (MPHP) for Swan Hill Rural City and the shires of Gannawarra and Buloke. These three municipalities are located in the north west of Victoria in the Mallee. This was the first time the three Councils had developed a joint MPHP. The project involved consultation with key community organisations, service providers and the conduct of public forums as well as research of public health issues relevant to the three municipalities. A common approach to the overall plan was achieved, with individual attention to each local government area. The MPHP was integrated with the Southern Mallee Primary Care Partnership Community Health Plan.
  • Development of a joint Municipal Public Health Plan (MPHP) for three local government areas in regional NE Victoria. The project involved consultation with key community organisations, service providers and the conduct of public forums as well as research of public health issues using a variety of health and population databases. A common approach to the overall plan was achieved, with individual attention to each local government area. The MPHP was integrated with a broader Community Health Plan.
  • Facilitation of the development of strategic, corporate and operational plans for a variety of organisations and agencies providing a range of supports to the community, including community health services, disability services, youth services, children’s services, SAAP services.
  • Feasibility study and business plan for a multi-purpose medical centre in Rutherglen, Victoria. This project was conducted for Glenview Community Care and involved assessing the feasibility of co-locating community health services, General Practitioners, Indigo Shire’s community services such as Maternal and Child Health and Youth Services, and visiting specialist services. We consulted with all key stakeholders through surveys, forums and interviews. A business plan was developed collaboratively with the core stakeholders and a submission made to the commonwealth for funding.
